New Resident Admissions

The Lukas Community is a certified, state licensed Assisted Living Facility in NH.  Residents must have a Developmental or Intellectual Disability and be 18 years of age.  

We encourage families who are interested in The Lukas Community to contact us to learn more about our programs, residential life, and the State funding sources available.  

If Lukas seems like it may be a good fit all around, these are the steps:

✅ We like to start with a tour so you can see it for yourself; experience the land and the houses and say hello to our staff and residents.

✅ Fill out an application; this is our introduction to the person who is applying to live in The Lukas Community. It lets us know basic information, as well as medical and other needs, hobbies, interests, and concerns. 

✅ The next step is a one-day trial visit, where the prospective resident has an opportunity to join in programs, enjoy meals, and meet the staff and residents at Lukas.

✅ After this visit, our administrative team will touch base with the family to see how the day visit went from their perspective, and share how our team felt the prospective resident did as well.

✅ After the visit, you may be invited for a two to four-week trial visit, during which time the prospective resident will fully experience life at Lukas. If the resident and our community are a good fit for each other, we would extend an invitation to join us.  We would work with your team to explore the financial options (private pay vs. State Medicaid funding), to ensure Lukas is a long-term residential solution.

During the trial visit, the prospective resident would live in one of our four households, take on household responsibilities, share wholesome meals, and spend the day in productive contribution to our community.

Trial visits last two-four weeks, and Lukas requires $2000/two week stay to fund your stay. Throughout the entire admission process, we look for basic compatibility. We try to determine the individual’s needs and strength, and assess what our capacities are to meet their needs. We look at what they may bring to our community in terms of sociability and other qualities, as well as what they can offer and contribute to our work life. We will assess their care and emotional support needs, as well.
